Status of Domain Name Service

  This form is for requesting cancelations of The World Kiosk
  services.  This form *DOES NOT* cancel your Internet Access through
  the World (e.g. account@world.std.com, your dial up service, your
  login account).  If you would like to cancel your World Account with us,
  please contact office@world.com.  The Kiosk services encompass a
  number of features each of which is driven by the Domain Name Service
  (DNS) as listed at the InterNIC.  The Domain Name Service is the
  function provided by the InterNIC registry which defines an Internet
  domain name by listing the DNS server.  The DNS server is usually the
  same as the provider of the ISP services.  For customers of The World
  Kiosk services, DNS is included at no additional fee unless the
  customer has made other arrangements for their DNS service.

  Your Kiosk Agreement is valid until Software Tool and
  Die receives a Kiosk Cancelation Form sign by the owner of the
  Kiosk Services. All monetary charges pertaining to the Kiosk Service
  will accrue until this form is received.  Please complete this form
  and fax it to 617-739-0914 to the attention of Kiosk Services.

  Since modification to a DNS record will affect the delivery of the
  Internet services, the following sections needs to be reviewed
  carefully and check one of the two following:

  ___  Delete Domain Name from the InterNIC database

         Delete the domain name from the InterNIC's database of
         public Internet registry.  This should only be requested if
         all Internet services for this domain name are to be
         terminated and the Internet Domain Name registration removed
         from the InterNIC.

  ___  Cancel DNS services provided by The World

      	 Domain Name Service for this domain will no longer be provided by
      	 The World and the InterNIC registry will no longer reflect The
      	 World as the primary server. 

         SPECIAL NOTE: To TRANSFER a domain name, the new ISP needs to
         place the request for the update to transfer DNS services
         directly with the InterNIC.  

Kiosk Services - These are the services that you purchase that allow
your domain name or website to function.  If you have a domain name,
you have one of the following services.  Please check the service
which you are canceling.  If you are unsure as to which service you
have, please contact the Kiosk Department.

        - Virtual hosting of www.domain.com will no longer 
          be provided by The World.

        - All files and data related to this website have
          already been removed.

        - Mail services for this domain name will no longer
          be handled by The World.

  ___ Cancel Kiosk Custom Web Address

        - Virtual hosting of www.domain.com will no longer 
          be provided by The World.

        - Mail services for this domain name will no longer
          be handled by The World.

  ___ Cancel Kiosk Website 

        - The website http://www.std.com/website-name will
          no longer be provided by The World.

        - All files and and data related to this website have
          already been removed.

  ___ Cancel Kiosk Mail Box Service (KMBS)

        - Mail services for this domain name will no longer
          be handled by The World.

As the owner of The World account listed above and with authority for
the domain name specified, I acknowledge the submission of this Kiosk
cancelation request.  I understand that to cancel a World dialup
account, I need to contact The World billing office directly via email
